Position the patient.

Elevate the head of bed 30° to reduce aspiration risk. Suction any accumulated secretions before starting.

Inspect the oral cavity.

Check for lesions, bleeding, dryness, debris, and denture fit. Document findings.

Cleanse.

Use a soft toothbrush or suction swab with approved cleanser. For VAP prevention, use 0.12% chlorhexidine per protocol.

Comprehensive oral care reduces the risk of ventilator-associated pneumonia (VAP). Bacteria colonize the oral cavity and aspirate into the lungs — routine cleaning with approved products is a core VAP prevention intervention.

Are suction toothbrushes reusable?

Most suction oral care devices are labeled for single-patient use and are disposed of at discharge or end of the therapy course. Check the product label for reprocessing instructions.

Is chlorhexidine safe for all patients?

0.12% chlorhexidine gluconate is the standard for VAP prevention but can stain teeth with long-term use and is not recommended for certain populations. Follow facility protocol and physician orders.

How often should oral care be performed?

For ICU ventilated patients, oral care is typically performed every 2–4 hours. For general inpatient care, at minimum twice daily with toothbrushing and as needed.
